Prior Authorization (PA) Supplementary File
Do you need help generating the new prior authorization (PA) supplementary file? The file includes:
- Prior Authorization Group Description (Name) - Generated by Zynchros
- Drug(s) Cover by the PA - Generated by Zynchros
- Covered Uses - Plan sponsor supplied
- Exclusion Criteria - Plan sponsor supplied
- Required Medical Information - Plan sponsor supplied
- Age Restrictions - Plan sponsor supplied
- Prescriber Restrictions - Plan sponsor supplied
- Coverage Duration - Plan sponsor supplied
- Other Criteria - Plan sponsor supplied
For each of your formularies, our professional services group will:
- Provide you with a template generated from your formularies, pre-populated with each Prior Authorization group and drug information
- Deliver a simple interface to edit your text fields and,
- After you’ve completed the forms and returned the file(s) to us, we’ll generate a Prior Authorization Supplementary file for each of your Zynchros Part D 2009 formularies
The supplementary file will include each PA group in the formulary, the list of drugs within each PA group on formulary, and the text fields you provided, all in the required tab-delimited flat file format and meeting the field size limits CMS mandates.
